JOB SUMMARY:
Preserves, supports, and restores health and wellness by practicing at the highest level of the Registered Nurse licensure; collaborates with providers and multidisciplinary care team members to provide care coordination across the continuum.
Assesses and supports physical symptoms, provides psychological support to patients, friends, and families, mentors assigned team members, promotes best practices, and always maintains professional disposition.
QUALIFICATIONS:
Graduate of an approved school of professional nursing.
RN with a current license to practice as a professional nurse in the State of Michigan.
BCLS, ACLS and PALS certified.
TNCC certified within one year of hire.
Experience working in Medical-Surgical and ED areas.
Knowledge and experience in documenting in an Electronic Medical Record (EMR).
Complete understanding and compliance with all HIPAA regulations.
DUTIES & RESPONSIBILITIES:
Assesses the patient systematically utilizing verbal and technical nursing skills to establish a basis for patient care.
Utilizes the assessment data to plan appropriate nursing intervention.
Develops problem list and care plan on new admissions.
Records nursing needs on care plan and updates care plan to assure continuity of care.
Integrates teaching and discharge planning into the care plan.
Encourages patient and family to participate in care planning and discharge planning.
Coordinates referrals with physicians, outside agencies and other departments.
Recognizes needs of inpatients vs.
swing bed patients.
Oversees and coordinates daily care of assigned patients during shift.
Formulates a plan of care reflecting pertinent physical, psychosocial and past medical history and uses this plan as a tool in providing patient care.
Patient care and plan of care are based on knowledge of professional standards of nursing practice and adheres to hospital/departmental policies and procedures.
Follows nursing protocol in assuring proper format is in compliance with standards of care.
Evaluates the nursing plan of care and the patient’s response to the nursing intervention on a continuous basis.
Reassesses short term objectives and initiates discharge planning based on the evaluation process.
Identifies the learning needs of patients and families and meets these through the teaching/learning process.
Documents teaching and evidence of learning in the patient’s record.
Understands specific needs of patients of all age groups and is able to care for them and teach them appropriately.
Coordinates referrals with physicians, outside agencies and other departments.
Initiates an effective working relationship by utilizing interpersonal communication techniques with health care delivery team and coordinates patient care with staff assistants (LPN’s, CNA’s and Ward Clerks).
Maintains an appropriate communication system through documentation and interaction with patient, significant others and members of the health care delivery team.
Acts as a patient advocate.
