Summary: Under the general supervision of the Director of Social Services, the MAT/ Medical Social Worker is responsible for participating in the facilitation of patient and resident movement throughout the continuum of care within the facilities and in the community.
This includes provision of social services to patients, residents, their families and significant others to enable them to deal with the impact of illness and related conditions.
The Medical Social Worker arranges to meet these needs through existing social service programming and appropriate referrals to community Social Service agencies, various community resources, and professional services.
MAT-SW is responsible for providing support to MAT patients through counseling and case management services.
Responsibilities: QUALIFICATIONS/EDUCATION: Masters level degree from a C.

W.
E.
accredited Social Work program which includes concentration in practice skills and theoretical foundations.
CADC/ CAADC or ability to obtain within one year of employment.
Registration with the Michigan Department of Consumer and Industry Services at the minimum level of a Limited Licensed Social Worker or meeting the criteria to be so registered.
Two years’ experience in providing direct services to patients/residents in a health care setting preferred.
Ability to demonstrate knowledge and awareness of requirements and regulations regarding delivery of social services as set forth by Medicare, MDCH, and JCAHO.
Ability to demonstrate knowledge and awareness of community social service resources, ability to utilize such resources on an as needed basis, and ability to develop liaisons with community resource personnel.
Medical knowledge and terminology as it relates to delivery of social services.
Ability to maintain effective relationships with patients/residents, significant others, health care team members (including co-workers, physicians, and consultants), supervisors, management, and other consumers.
Willingness to work with flexible scheduling to complete essential job functions.
Must be able to move intermittently throughout the day.
Ability to communicate both orally and in writing with a wide range of people.
Able to facilitate multi-disciplinary work groups.
Must have the ability to work independently and organize several projects.
Must be able to speak the English language in an understandable manner.
Must be able to cope with the mental and emotional stress of the position.
Must be able to see and hear, or use prosthetics that will enable senses to function adequately to assure that the requirements of this position can be fully met.
