Duties Major Functions Clinical: Assesses inpatient and outpatient pharmacy programs from a cost and outcome perspective.
Maintains an effective quality assurance/improvement program for all inpatient and outpatient pharmacy programs.
Functions as the subject matter expert advising pharmacy and hospital staff on pharmaceutical care policies in the medical center.
Assesses clinical and operational pharmacy programs from a cost and patient outcome perspective.
Assists in the development, implementation and evaluation of programs which facilitate clinical activities including: Drug Use Evaluation, Quality Management and Pharmacy and Therapeutics Committee decisions.
Evidence based evaluation of therapeutic alternatives utilizing the most efficacious, least toxic, and most economic pharmacologic treatment modalities.
Coordination of patient information from all healthcare providers.
Provides the framework, philosophy and environment for effective pharmacy staff participation in the following clinical activities: Evaluates orders for appropriateness and drug interactions, allergies, and adverse reactions.
Intervenes with physicians and other staff members for rational, safe, and effective drug therapy and documents interventions.
Knows and follows formulary guidelines and ensures therapeutic and cost effective alternatives to non-formulary drugs.
Monitors drug therapy and provides information to other health professionals.
Accountable for medication reconciliation according to procedure.
Provides age-appropriate care to adult and/or geriatric patients.
Provides appropriate patient consultation for all new prescriptions.
Inquires about side effects and patient questions for refill prescriptions.
Evaluates patient's ability to understand instructions.
Provides oral and written instructions to promote patient medication adherence.
Performs drug specific pharmacokinetics.
Manages anticoagulation therapy.
Renews and orders medications per Scope of Practice with yearly peer review.
Critically reviews non-formulary drug requests for appropriateness, safety, and cost-effectiveness and may approve according to criteria for use.
Conducts prospective/retrospective drug use evaluations of high-use, high cost, or high-risk drugs as requested.
Converts patients to appropriate medications where indicated.
Changes the patient's drug therapy, order laboratory tests, and performs the necessary follow-up to ensure medication conversions are safe and effective.
Assists with manufacturer medication backorders and recalls including communication with providers and recommendations for alternatives.
Administrative: Responsible for the overall operation and scope of pharmacy activities performed in inpatient and outpatient pharmacy programs in Iron Mountain and area CBOCs Responsible for the supervision of all inpatient and outpatient pharmacy personnel in Iron Mountain and area CBOCs.
Evaluates performance of inpatient and outpatient programs to ensure the quality of services provided are of the highest standards.
Assesses workload and adjusts staffing to meet the needs of inpatient and outpatient services.
Provides justification for additional resources as needed.
Supervises, documents performance, and counsels technicians and pharmacists.
Completes annual performance appraisals, competency assessments, award recommendations, etc.
within dictated time frames.
Supervises pharmacy operations and clinical areas in the absence of the existing supervisor or manager and oversees management of unexpected staff absences in consultation with other pharmacy managers, and as directed by the Chief.
Works collaboratively as a member of the pharmacy management team: Interviews, evaluates, and selects new employees according to current federal laws and VA policies regarding EEO and veterans preference.
Formulates and implements effective and efficient policies and procedures.
Develops and maintains a cooperative working relationship with other departments such that the overall activities and scope of pharmacy services rendered are carried out efficiently Consults with the applicable medical chiefs and/or the Chief of Staff regarding clinical and/or administrative problems identified in inpatient and outpatient pharmacy services.
Responsible for developing and maintaining competencies and required training for pharmacy staff in these areas.

Education: Graduate of an Accreditation Council for Pharmacy Education (ACPE) accredited College or School of Pharmacy with a baccalaureate degree in pharmacy (BS Pharmacy) and/or a Doctor of Pharmacy (Pharm.

) degree.
OR Graduate of a foreign pharmacy degree program if you are able to provide proof of achieving the Foreign Pharmacy Graduate Examination Commission (FPGEC) Certification, which includes passing the Foreign Pharmacy Graduate Equivalency Examination (FPGEE) and the Test of English as a Foreign Language Internet-Based Test (TOEFL iBT).
Licensure: Full, current and unrestricted license to practice pharmacy in a State, Territory, Commonwealth of the United States (i.
e.
, Puerto Rico), or the District of Columbia.
Grade Determinations: Specialized Experience, GS-14: In addition to the GS-13 requirements, you must have at least 1 year of experience equivalent to the next lower grade level, GS-13.
This experience may include, but not be limited to: Responsible for coordinating multiple programs in pharmacy service in a medical facility.
Serves as the Acting Service Chief in the absence of the Service Chief.
Develops and maintains a system of internal reviews that ensure service programs operate at a satisfactory level of performance and in compliance with regulatory and accrediting bodies.
Has extensive responsibility for pharmacy utilization of resources and budget.
Makes selections, assigns personnel, and serves as a mentor to help employees develop their full potential.

Physical Requirements: The work is performed within the pharmacy and the medical center, and at times may require travel.
The incumbent observes safety precautions while working with a variety of equipment.
